  Testing offered on the first and third Monday of each month from 3:00 p.m. to 4:00 p.m. and the second and fourth Tuesday of each month from 8:30 a.m. to 9:30 a.m.
• Marshfield
  By appointment only, the second Monday of each month from 9:30 a.m. to 10:15 a.m.
