COVID-19 Information

Norwood Health Center is committed to protecting our residents and staff against COVID-19.

We have procedures and precautions in place to protect everyone.

Visitation in the building must be scheduled in advance of your visit. Anything that can be done virtually is strongly recommended.

We screen all individuals that enter our building, and you may be asked to have a rapid COVID test if you are going to have patient contact.

Vaccines

Norwood Health Center is providing vaccines at their location by appointment only and is following the State and Federal guidance on vaccination groups.
To be placed on a waiting list please call 715-384-2188 extension 3238 and leave your:

  • Name
  • Phone Number
  • Date of Birth (DOB)
  • How you meet the requirements for the vaccine presently (ie: over the age of 65, health care worker, etc).

You will be put on a waiting list. If you have the opportunity to get the vaccine prior to us returning your call, you should take the opportunity. Our weekly vaccine allocation is determined by the Federal and State government and currently the demand for the vaccine far exceeds the supply available.

We are attempting to service Wood County residents, and if you reside outside of Wood County, please try and get the vaccine from your local provider.

There are two reasons you should not get the vaccine:

  1. If you have had a reaction to a previous dose of the vaccine or
  2. are allergic to any of the properties in the vaccine.

IF you have any chronic medical conditions, please confirm with your doctor that they recommend you get the vaccine. Our vaccine administration team is unable to determine that for you. We will hold no liability for you not seeking that advise prior.

We are administering the Moderna vaccine at this time, but this may change as we do not control what brand we receive.

You must wear a mask the entire time you are in the building and you must wait a minimum of 15 minutes after each dose of the two dose vaccine.

Please bring your completed Consent and Screening Form (2 forms) with you for your vaccine appointment and make sure to read and understand the safety information provided below.
